All new patients are asked to bring the following:
- Prescription/referral from you doctor
- X-rays, and reports, and/or MRI CD and/or reports, any other test results
- List of your medications
- Valid ID/Drivers License
- Insurance information
- Medical and surgical history information
You can print following following forms and bring them with you for your first appointment. If you do not have an access to a printer, please arrive 15 minutes earlier to allow enough time for the paperwork.
